The main contribution of this special issue is to present evaluation studies involving large-scale experiences of implementation of positive parenting programs delivered through home, group-based, and on-line formats in Spain. Two research questions were addressed: (1) what factors affect implementation; and (2) for whom and under which implementation conditions the programs lead to positive outcomes. Target populations were mainly families from low and middle socioeconomic backgrounds, and parents at psychosocial risk attending family support services in need of improving their parenting skills. All the programs fall under the umbrella of the positive parenting initiative launched by the Council of Europe, are evidence-based, follow a collaborative schema with national, regional, or local authorities, have multi-site implementation, and are supported by highly experienced researchers from Spanish universities. Special attention is given to the program adaptations to different contexts, the profile of parents who benefited most from the programs, analyses of the implementation process, and the assessment of parenting programs in the community. The information provided will help to increase our knowledge of evidence-based parenting programs in Spain, their implementation processes and results, and the future challenges that need to be addressed to continue the current expansion of evidence-based parenting programs.

Family prevention programs need to be evidence-based in order to guarantee the success of their implementation. The Family Competence Program (FCP), a Spanish cultural adaptation of the Strengthening Families Program (SFP), has developed different measures and processes to gauge the quality of the implementation. This article is dedicated specifically to two of these measures: the evaluation of the facilitators and the assessment of the family engagement techniques. For evaluating the facilitators, a Delphi technique with experts and professionals is undertaken. For assessing the family techniques, both self-evaluation of trainers and evaluation by families are used. Finding underpin that, in the case of facilitators, is important that, after to skills and experience, they need to understand the theory of change of the program. In the case of family engagement techniques, more detailed, comprehensive talks, discussions and group activities lead to better family engagement outcomes.

Objetivo: Identificar estructuras estandarizadas para la formulación de preguntas clínicas en el marco de la práctica basada en la evidencia. Método: Se realizó una revisión de la literatura. La búsqueda bibliográfica se efectuó en las bases de datos MEDLINE, CINAHL, EMBASE y LILACS, y en el buscador académico Google Scholar. Se efectuaron estrategias de búsqueda sensibles y acordes a cada base de datos. Límites de búsqueda: intervalo de tiempo (enero de 1995 a abril de 2015), idioma (artículos en inglés). Se usaron palabras clave libres y descriptores del Medical Subject Headings (MeSH) pertinentes: evidence based practice, question, formulation, well-built question, framework. Resultados: Se encontraron 10 manuscritos que aportaron el diseño original de estructuras para la formulación de preguntas clínicas en el ámbito de la práctica basada en la evidencia. El modelo PICO es la estructura más conocida y comúnmente utilizada en investigación cuantitativa y de él derivan los modelos PICOT, PICOTT, PICOS, PIPOH, PECORD, PESICO. En el campo de la gestión sanitaria la estructura ECLIPSE se erige para formulación de preguntas relacionadas con la gestión. Por último, para la búsqueda de evidencias cualitativas se han configurado los modelos SPICE y SPIDER adecuando sus componentes al fenómeno cualitativo. Conclusiones: Estos modelos estandarizadas se comportan como un instrumento idóneo para guiar la estrategia de búsqueda y delimitar el área de interés. Dada la gran variedad de piezas que integran las estructuras, su conocimiento exhaustivo acrecienta sus usos potenciales. Estas estructuras no deben ser consideradas como una guía rígida a la que ceñirse ineludiblemente.

Introduction: The nutrition of very low birth weight (VLBW) infants is aimed at promoting a similar growth to that occurring in the uterus. However, in practice this is difficult to achieve and extrauterine growth restriction is frequent. The current tendency is to avoid this restriction by means of early parenteral and enteral nutrition. Nonetheless, uncertainty about many of the practices related with nutrition has resulted in a great variation in the way it is undertaken. In 2009 and 2011 in our hospital there was an unexpected increase in necrotizing enterocolitis. To check to see whether our nutrition policy was involved, we undertook a systematic review and drew up clinical practice guidelines (CPG) about enteral feeding in VLBW infants. New considerations about the duration of the fortification and the use of probiotics have led to an update of these CPG. Methods: A total of 21 clinical questions were designed dealing with the type of milk, starting age, mode of administration, rate and volume of the increments, fortification, use of probiotics and protocol. After conducting a systematic search of the available evidence, the information was contrasted and summarized in order to draw up the recommendations. The quality of the evidence and the strength of the recommendations were determined from the SIGN scale Comment: These CPG aim to help physicians in their decision making. The protocolized application of well-proven measurements reduces the variation in clinical practice and improves results.
